First Visit

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Please arrive 15 minutes early on the day of your first visit.  Often, treatment can be done on the same day. A more complex medical history and/or treatment plan may require an evaluation and a second appointment to optimize treatment outcomes.

Please assist us by providing the following information at the time of your consultation:

  • Insurance Card and Drivers License
  • A list of medications you are presently taking.
  • Please click the link below to print and complete the our registration and medical history forms.  Bring the forms with you along with your insurance card and drivers license on the day of your appointment.

IMPORTANT: Patients Under 18 must be accompanied by their legal guardian.  The legal guardian must have authority to approve medical care and will be held financially responsible for all care rendered.

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.).
